As a Clinical Pharmacist, you will be responsible for providing expert advice on medication therapy management, ensuring that patients receive the most appropriate medications for their conditions. You will work closely with physicians, nurses, and other healthcare professionals to optimize patient care, prevent medication errors, and improve overall health outcomes. Your role will also involve educating patients on medication usage, side effects, and safety precautions.
Duties and Responsibilities
- Collaborate with healthcare providers to review and recommend appropriate drug therapy options for patients;
- Assess patients’ medication procedures, providing advice on drug interactions, dosage adjustments, and potential side effects;
- Participate in rounds with physicians and other healthcare team members to discuss patient care plans and treatment options;
- Ensure accurate medication prescriptions and dispensing, confirming that medications are appropriate, safe, and effective;
- Monitor patient records for medication adherence, side effects, and therapy outcomes;
- Manage drug therapy monitoring for specific conditions such as hypertension, diabetes, and infectious diseases;
- Counsel patients on proper medication use, including dosages, potential side effects, and the importance of adherence to their treatment plan;
- Answer patient queries regarding medications and provide support in navigating pharmaceutical services;
- Ensure compliance with medication safety standards, regulatory requirements, and ethical practices;
- Stay updated on the latest developments in pharmacology, drug therapies, and clinical guidelines;
- Participate in ongoing education and training programs to maintain professional competencies;
- Oversee the management and procurement of medications and pharmaceuticals, ensuring availability and proper storage;
- Work with the pharmacy team to maintain inventory records and ensure the timely replenishment of stock.
Job Requirements
Academic Qualification
- Bachelor’s science degree(BSC) in Pharmacy/Pharmacology, with valid Professional license;
Additional Skills
- Strong knowledge of pharmacology, drug interactions, and medication management;
- Excellent communication and interpersonal skills with the ability to work in a collaborative environment;
- Ability to work independently, handle multiple tasks, and make informed decisions under pressure;
- Familiarity with medical software, patient management systems, and electronic health records;
- Strong attention to detail and commitment to maintaining patient safety and confidentiality.
Work Experience: 2+ years and above in similar work
